If he could be hypoglycemic, that's not an uncommon problem with small dogs. Give him a little honey, that should bring his blood sugar back up.

This puppy is seriously depressed! 10 hours is much too long for a puppy this age to be alone! He should still be eating 3 meals a day at this age. He's used to being with his mother and/or littermates. Now he's alone in a quiet house and dogs (especially puppies) hate being alone. He needs lots of attention including during the day. You may have to pay someone to come in and spend time with him while you're at work or even take him with you if possible. This breed is very sensitive and has a strong need for companionship: I've seen them pine away like this and what saved one was my carrying him around inside my sweatshirt for two days until he started to show an interest in life again. This little guy is hurting and he needs your help. If you absolutely MUST leave him alone for several hours, you can use a hot water bottle or microwaveable heat pad (never an electric heating pad) under a blanket and a windup alarm clock or other source of soft noise to help keep him company.
Also, if he's not getting any fluid it can be dangerous and even fatal. Tiny breeds can dehydrate quickly so you have to get something in to him even if you do it by hand every hour.

We had the same problem with a puppy we adopted from the pound. Sometimes that is just the nature of the dog and you have to accept that. Even then, the pup will start to warm up to you after a while. You will start to see the tag wag a little bit more each time you come home. As you vet said, "Let him have some time adjust to the new home." It took my pup almost 6 weeks to start to warm up to my friends when they would come over.

puppies miss their siblings or other play mates and they need to adjust to a new life. he may not know how to entertain it self,, so it lays around. and if it is not active then that could be the reason it is not taking any food or water. little puppies take a little amount of water that you may not notice the different in the water container. and just buy him some toys and safe [ safe] chews.he is needing to get over the lost of one home and getting use to another,, good luck. !!

Doxie's are very needy of their humans... they need a lot of your time and attention. 10 hours is WAY TOO LONG for you to be leaving your puppy. You need to make arrangements for someone to spend time with your pup during the day if you are unable to. Doxie's don't do well with their owners gone a lot at all. This is something the breeder should have been discussing with you to determine if this breed was suitable for you and your lifestyle.

Give the puppy some karo syrup or honey or nutrical if you have it. If he isn't eating his sugar has probably dropped dangerously low and it can kill him. That is why he has been shaking so bbad. You also need to take him to another vet. He may be very sick. If this pup doesn't get help, it is going to die.

Bring him back to the vet tomorrow. Behavioural problems are often signs of illness. Perhaps try another vet. In the meantime, just give him lots of love and support ...

As the vet said, give him time to adjust.
I have to add also though...ten hours is WAY TOO LONG to leave a puppy alone and milk is NOT good for him.

just give it some time to trust you, and hold and love it as much as you can, in no time it will play and trust you, it may need to sleep with you to feel safe
