I don't know why guys freak out about this sort of thing. BE YOURSELF! Shes going to find out the real you sooner or later. Might I well start now. Just say "Hey, I really like you and I was wondering if you wanted to see a movie this Friday." simple and to the point.

Depends on the situation. Your best bet is to say, "Hey, I think it'd be cool if we could hang out sometime, what do you think?" or something like that. If she says she thinks it's a splendid idea, then tell her you'll call her and then call her later than night (not after 9:30, it will make it seem like she's last on your priority list - not a good sign!) and tell her what you want to do.

Bro, say something like, I've liked you for a little while, and you're awesome, I love hanging out with you and you make me happy.
Then just be like, will you be my girlfriend, or ask her out on a date just so you two can see where things go, unless you've done it before.

before you ask her get to Know her a little walk up to her and say hi my name is and after you get to Know her a little then ask her out on a date by saying I was wondering if maybe you would like to continue this conversation sometime
