use 12 inches to the center of your bolt holes on the floor. center your drain inbetween and you'll be ok.
the side to the wall should be 16". gives you room for tank and whiping.
put your water feed on the left side looking at the tank and in line with the ballcock to the tank.

you're in business.

Usually, the tank is tight to the wall. If you are replacing the toilet, you kind of have to scope out the size of the base of the toilet relative to the hole in the floor. There are various sizes of toilet bases. I'd measure the old toilet base and replace it with a toilet that has a base the same or nearly the same size as the old one. Good luck. I think the center of our holes are ABOUT 15 inches from the wall.
