A call to the vet is definately in store here. But, I would assume that you have already done that. Is the blood red? If so, it is probably not much to worry about. The diarrhea on the other hand can be serious.
I have had this before with my dog and the vet told me not to feed him for 12 hrs. then give him a bland diet (boiled chicken, hamburger mixed with some rice) If he won't eat that then your vet should have some food that would work. I did this until he had a normal stool.
But, Please call the vet ASAP.

When you asked this question...I think you already knew the answers you would get...so I'm just going to state the obvious.
Your dog needs professional medical care. Contact your vet or take your beloved pet to an Emergency Pet Care facility immediately. Talk to the vet about the cause of the medical issues and about different options of treatment. You may need to change your dogs diet...there may be medications that can help your dog with these issues...but until you talk to a professional who is familiar with your dog's history, you will just be getting a lot of opinions.
Ask your vet about the suffering of your pet. Will treatments alleviate the suffering or just prolong it? You may not get the answer that you want to hear, but as long as you have a vet for your dog that you trust, he will give you the knowledge and advise that you need to make the decisions that need to be made.

All I know is that a dog will eat grass if it has an upset stomach just like us humans we take pain killers for upset stomachs the dogs eat grass. I doubt it the blood in the stool is because of the grass eating has I have two dogs my self who chop away on grass and there stool never shows blood in! It sounds more like because they have eaten pork chops bones when they have chewed this the bones have gone into small splinters resulting in your dog's finding them Delicious to chew away on. The blood in the stool is most probley caused by the bones scratching on something inside the dog causing a minor bleed. I wouldn't worry to much just keep a eye on them for the next day, if there is no change in the stool and its still showing blood, Then please please please don't hesitate to contact your emergency vet. Taking the dog's to the vet is the only suitable option for this, has the vet will take a blood test to see if the problem is anything more serious e.g the kidneys. or they even might X-ray the dog to see if they can see the problem more clearly. Take the dog to the Vet asap! Blood and mucousy diarrhea at any age but especially at that age could be something serious. There are too many things it could be. From something more easily treated, like hookworms, to serious diseases like pancreatitis or cancer.

Especially since he's had issues before, I'm thinking you must have a vet who knows his history. Call him. If he doesn't respond to weekend calls - then find an emergency facility that is open. Call them - let them decide if you have the luxury of time to wait until tomorrow to seen your vet.
He's 12 years old - I know you want what's best for him. His health and safety must be how you make your judegment.

I would definitely consult a vet, especially because of the problems he has had.
However, my 7 y/o dog had this last weekend and I watched her closely for recurrences, but I think my dogs problem was because she refused to go out in the ice and rain to use the bathroom, it was only in the "present" she left in the hallway when her bowels wouldn't hold anymore.

Hm - could he have gotten into something - like, food cooked with onions? Feed canned pumpkin (unsweetened - just straight pumpkin!) or chicken and white rice. If it doesn't stop in the next 12 hrs, please go to your vet.
