I have made these for productions. A pillow usually looks like just that. Here is what I have done.
At a craft store I bought a hard Styrofoam ball about the size of a soccer ball. Using a sharp serrated knife, I cut it in half, and then carved it to a slight concave on the side that sat against the actress's belly for a smooth fit.
Once it was the right size for her 'bump', I wrapped it in a thin sheet of cotton batting. (Cheap at craft stores also.)
Then, I took a woman's slip in a flesh tone and wrapped this tight around the whole thing. I got this at a thrift store. A camisole would work too, but get one with some stretch to it. It needs to fit tight.
The actress wore a long tight fitting tank top under her costume, and just slipped the belly in. We even used a small piece of cotton to fashion the right belly button.
Don't forget to stuff her bra a bit to complete the pregnant look.

They're a little expensive though. So it will depend on the nature of the production your friend is in.
