You have an HD Ready TV so that is one thing out of the way.

Yes there are channels being broadcast as HD at present, both on Sky and NTL/Telewest. You will need to subscribe to either Sky HD or Telewest's TVDrive service in order to obtain the hardware that is capable of processing HD.

The HDMI cable is simply a cable capable of sending the Video/Audio data from one HD device to another. Don't worry about it, it's a cable - just plug it in and forget about it.

Get your HD box, plug it into your tv with the HDMI cable and job's a good 'un.
