"Underweight" and "overweight" according to whose standards? Every culture has different stands of what is acceptable and what is not. Most people in the world are underweight according to American norms, but most Americans are overweight if you go by the averages for other countries.

In less developed countries, more people are underweight. In developed countries, like the US, Canada, and many European countries, the numbers of overweight people are much higher. Even in Asian countries where a Western diet is becoming more common, more people are overweight.

But for the entire planet? They probably balance out.

In Western countries the diseases related to obesity and over-nutrition are at epidemic levels. By 2010 Type 2 Diabetes and related problems of this disease will be the biggest killer in the Western world out weighing (pardon the pun) heart disease.
Third world countries are suffering from diseases relating to under-nutrition such as malnutrition.
Both of these extremes are simple metabolic abuses that in most cases can be easily rectified.
