That is an interesting question because I do not think I have ever uttered the words: "Would you like to go out on a date?" I have always suggested that we attend some event together or just suggested a bite to eat or seeing a movie together.
Just be natural and do not think about "asking out on a date", but suggesting that the two of you do something together. It sounds natural and less pressured than if you are on a "date".

You should go up to her ask her out. If you want to make it special then send her a rose with a card asking if she will go out with you.
