My daughter didn't receive a letter last year and didn't fill out a W-4 form, I would like to know what is the W-4 tax form and she has thirty days to reply.

Hello, she is recieving the W-4 form because she is working now and they need that info to process the number of exemptions she requests in there and thus figure out what goes in her W-2 (the form she'll use to declare taxes).

The number of exemptions in her W-4 will determine how much tax they will keep from their paycheck every month. Since she hasn't filled a W-4 before, right now they are assuming she has 0 or 1 exemptions, that's why they are saying that if you want exemptions you must return the W-4 form. So, if she explicitely fills the form out, then she could request more...Read a bit on tax exemptions to understand how many she should be asking for based on her situation and status.
