ONLY if the cosmetic surgery is to correct a defect or scars from an injury or other surgery.

Insurance does not cover surgery that people get just because they aren't happy with themselves.

no no such thing sorry it all depends on what the company refers to as cosmetic a nose job for example would not be paid for. If you google it the first one there is helpful

If the surgery is elective, then let me know if you found one. But usually, no for elective cosmetic surgery.

If it's for reconstructive purposes - like correcting a cleft palate - then it probably will be covered. But if you want bigger boobs or a smaller nose or smaller butt or want the weight loss surgery - save your pennies because it's HIGHLY unlikely it'll be covered.
