I have a flight to Canada out of JFK at 8:00 am, I am staying on the upper west side, what is the best way to get to JFK by about 5:00 - 6:00 am?
(Note: I have looked at a lot of PT options and many do not start until 6:30 am)

I agree that using a cab is the best option for $45.
You could do the AirTrain option, but because it only runs every 20 minutes at that hour, you are going to be spending a lot of time getting there.
There is no traffic at that time, so the cab ride is quick and painless, albeit expensive.
My flight was 6:45am last week and a cab was worth it.
The subway runs at that hour as well, I was going to take the 6 to the E to the Air Train.
If you really want to do it - take the A and transfer to the AirTrain. It'll cost you $7 per person on a MetroCard. And take some time waiting for the trains and hauling your luggage up and down subway stairs...

Take the subway to Penn Station
Take the long Island Rail Road to Jamaica Station
Go upstairs and take the Air Train to JFK
Cost
subway $2.00 LIRR $5.00+ Air Train $ 5.00
The first train from Penn to Jamaica is 3:52 A.M. and they run about every 20 minutes after that
The Trip to Jamaica takes about 20 minutes
and another 10 minutes or so to JFK
the Air train runs like every 10 minutes or so
If you time it right you can get to the airport in under an hour.
When you buy your ticket at Penn get the Air train ticket also

maybe the taxi? or you can call a limo. limo is sorta cheap. it's prob. like $20-30
