Check the fluid level in the reservoir located under the rear hatch to see if it's full, if not, fill it. If it's full turn the ignition to the on position (not start), activate the rear washer located on the dashboard. Listen closely for the sound of the washer pump. If you hear it your fluid tube is blocked. If you hear nothing the rear washer pump has failed. A $12 part and easy enough to replace.

Rear wiper should be 1 size down from the front wipers. Most auto part stores that carry wipers will also have a book for reference. Double check to be safe.
Normally most rear window shooter fill jugs are located in the rear quarter panels. Open the hatch and look around the hatch opening, normally a small plastic cap to fill the reservoir like the front windshield shooters.

The water reserviour might be empty, check to see if it's not. The nozzle of the rear wipers might be clogged with some dead leaves or bugs or dust, try to clean it with a piece of long thin wire or a needle, When you run the winshields and try to spray water, do you hear the sound of the water pump ? if not it might be disconnected (the fuse) or the power connector to it. the last option to look for would be a bad motor where you have to change it.
Simply take the old windshield with you when you go to the repair man where he would be able to get a similar one (either from the serial number or the size of it :) )

The resevior is probably empty.
When you open the back hatch, on the inside of the van there is a cover to access the resevior.
I think they're 16". Not positive though. There are books at the auto parts stores that will give you the exact one.

you gotta find the winshield washer liquid container for the rear windshield then follow the line to make sure it is cleared. sounds stopped up

Res. for the washer has trash at the bottom and the pump can't suck the water, that's if you hear the pump working.
