Omega oils have been shown to work. You can take supplements, or increase the amount of oily fish you eat- your local fishmonger can advise on the best kinds, and probably help with recipes. It is also important to know why you want to focus in the first place- if you aren't interested in the subject at all, it is 100 times harder to concentrate.

You might suffer from gluten intolerance. It can make thinking very fuzzy; at least, I found it to be so in my case.
To test, try eating cereal of the wheat biscuit type - 3 at once - and see if it makes your thinking blur.
Doctors would know how to test more scientifically. Casein
intolerance, i.e a protein in dairy products, may also
affect clarity of thought.

Fasting food a day a week really increases one's powers of mental focus a lot I find - but you'll have to be very self-disciplined.

Eat and exercise regularly and healthily.

Fish oils are supposed to be good for concentration, I take Omega 3 (can get these at any supermarket).

Also if it is a serious problem have you tried going to your doctor, it might be a form of hyperactivity disorder.

firstly remove garlic and onions from your diet as these cause brain fog ,essentially they lower your concentration levels ,next remove any artificial sweetners from your diet ie aspartame ,acusulfame ,monosodium glutamate ,sodium saccharin ,next take plenty of hemp oil (found in most supermarkets these days ,this contains the richest source of amino acids and omega 3 ,6,9 oils ,if you do this it will take a couple of weeks for you to notice the difference in concentration levels ,aslo try doing some simple brain exercises ie crosswords ,sudoku etc ,within a month you should spot noticable differences

Sorry, what was the question?
Heheh I'm a moron I know!
Just focus on one task at a time, write lists of what you need to do, give yourself (realistic) goals but allow breaks so your brain can re-group.
