tell her you might screw up and that your VERY sorry if you do, and to forgive you. But still go.

Get a haircut the day before, make sure you cut all of your nails that day, shave (carefully) even if you don't have to often and DO NOT wear too much cologne or aftershave, wear VERY LITTLE, if any at all. Bring gum or, even better, mints (like Altoids) so your breath stays fresh.
When you FIRST see her tell her how pretty she looks (don't be vulgar though). Compliment her again a few times during the night. Be polite and act like a gentleman at ALL times. Dance with her many, many times, even if you're a bad dancer (they appreciate the effort). When slow dancing with her, look straight into her eyes for a little bit then whisper into her ear about how much you like her, how great she looks and how happy you are that she's there with you. Tell her how much fun you're having with her.
Don't eat too fast and be careful when you eat (if there's a dinner involved). DO NOT drink alcohol, it makes you act like a fool. Don't drink too much soda, the caffeine and sugar will make you hyper, which is not good. Stick to Sprite or just water.
Pay attention to her, do not make her feel like she's alone. Let her be with her friends when she wants to but remember, you are her date. Act like it.
Ultimately, relax and have fun. Don't get too stressed about it. I told you a lot of things but they're not too hard, right?
Have a great time.
