I repeat that I'm busy and can't talk right now, and tell them that I'll call them back later and then hang up before they have a chance to say anything else. If the phone rings again shortly after that, I let it go to voice mail; screen the call before I pick it up...if I decide to pick it up.

repeat in a more stronger tone, sorry i am very busy, then tell them a suitable time to phone back even if its the following day, if they still insist on talking then say i am going to put the phone down and do so if they continue, do not think you are being rude as they are rude too ignoring you like that.

If I'm home, I screen my calls, so I don't really run into that issue. I would suggest an inexpensice answering machine.
If you need a direct answer: To friends/family, I would say: "I really need to go, can I call you back?" If that was not enough: "I got to go, bye." To a stranger, I would just hang up.

I would politely interrupt them and tell them to either call you back in X minutes or tell them that you'll call them back when you're ready to listen.
As long as it isn't an important conversation (like my friend was in terrible car accident or my mother died yesterday), let them know that you're truly busey and, at that time, you can't offer them your full attention.
