There's this trainer at the gym who keeps calling this guy "muscleman".What does this term generally mean?

It means the guy has HUGE muscles. Very well built and strong.

A man with a muscular or brawny physique, guy who is a body builder and is in perfect shape. Someone in great shape who works out a lot. someone who does special exercises to develop the musculature.
