When someone gets indicted they have usually already been arrested and are in custody or out on bail.
A district attorney presents evidence to a grand jury when a person is charged with a felony and if the grand jury believes that enough evidence was provided to proceed with the trial that suspect is then indicted ( Formally Charged), however in the case of a sealed indictment evidence is presented to a Grand Jury without the suspects knowledge and once the grand jury decides to indict a warrant is issued for that party and usually the police department or law enforcement agency handling the investigation decides when to pick up the suspect. in most cases they do so shortly after the indictment is handed down.

The authority that had them indicted, usually a county attorney or attorney general! You are only indicted for things which can result in the loss of you liberty!
The Judge has no say into who is indicted! That is a separate jury! Some are formal and hold hearings and others are nothing more than a rubber stamp where the prosecutor gives an Offer of Proof!

The DA asks the judge for an arrest warrant, which the judge will issue. The question of when depends on law enforcement. If they know where you are, they'll come get you, if not they'll track you down.

Nobody really. After the paperwork is filed, signed, warrant issued, cops find you. Sometimes just like that, sometimes a week, month, set you up to bust you, few months.
