I don't think lack of sleep can drive someone insane.

Most people are already clinically insane because of a lack of sleep as many don't even get a sound 8 hours sleep at night. Severe depression really screws up sleep too. Somedays you can sleep 12 hours to 23 hrs and most other days less than 5. It would be difficult to determine when a person becomes clinically insane because lack of sleep affects people differently. There was a fight movie about a guy who couldn't sleep and he kept beating himself up, thinking that he was in a fight club.
When deprived of sleep the first organ to be seriously affected is the mind. Instead of dreaming, his dreams became very real to him. I liked the ending... he beats himself up in his boss's office and pleads for the boss to stop. Someone walks in and the employee now has his boss by the balls. So he gets to work at home and keeps his job. Might be something I would consider doing, haha.

I'm not sure.
I've been up four days straight several times when all the goats decide to have their babies at once and I'm running from pen to barn to stall to pen and back again. Kidding out 100 head of goats in a week eliminates sleep. At about four days though, even adrenaline can't keep me awake and I catch myself falling asleep leaning against a wall or when I'm on my knees trying to help a baby goat nurse. That's when I call in reinforcements and take a nap.
Sometimes though, the goats or cows string out having their babies and we'll spend months sleeping in two hour stretches between getting up to check and make sure everybody's all right.
Same thing irrigating. I've been up three days straight irrigating fields. More often it's a couple weeks catching two hour naps between trips to the fields.
Harvest and planting season are the same story. Sleep only when you can't stay awake any longer or you start making bad decisions.
Maybe I should move to the city and get some sleep!

if you have manic phase of bipolar disease, you would need to get help in order to get to sleep and begin to feel better (by medications) If it is from taking stimulant drugs like METH amphetamine, that is a big danger, causing insomnia, altered mental status and dangerous behaviour, also effects to the blood pressure and heart can kill the person. One cannot function very well after being up for perhaps 18 hours, driving is impaired. Every effort should be made to get at least a nap if you are in a field needing long hours of wakefulness. Your body will eventually take over, causing "micro-sleep" where you zone out into sleep for moments without any awareness that you did fall asleep-no warning. Therefore it is NOT safe to say, "I'll pull over if I get sleepy" when very sleep-deprived, as you will have fallen asleep before you get any warning. You must get the sleep BEFORE you get to that point, which means you must plan for it by calculating how long you've been up and forcing the nap to prevent a disaster. Sleep is not to be toyed with, your body needs it.
Thanks for the farmer's point of view, that is indeed hard work.

Staying awake does not make you clinically insane, brain disorders do not sleep deprivation

I don't think there is such a thing as "clinically insane." Insane is a term only used in criminal courts, now, I'm pretty sure.

If you are sleep deprived you become delirious, paranoid and you begin to hallucinate. That usually happens to people on meth or crack. It's the symptoms of a mental illness.
