Personally, I would write her a little note or an e-mail, or something, and just say, "Whenever you're ready, I'd like to talk. If there's something wrong, we need to work it out." And from there, just wait.
If she's really your friend, then she'll talk to you when she's ready. But the best thing I can tell you is to be patient with her.

A true friend would have come to you a long time ago and told you she was upset and why. You already are accepting guilt by wanting forgiveness without even knowing why!!!! Friendship is a 2 way street - I would not recommend seeking her approval. Let her go and find new more open people to make friends with.

There is only one thing you can do.
Say you think or know she is mad at you, your sorry for whatever it is and that you want to know what you did so you can clarify the situation.
The only way to fix this is by talking, if she doesn't want to talk about it, then there is nothing else you can do.
