The proper response would be:

Well - or - Not well

Technically, "good" or "bad" wouldn't be the proper response as they are adjectives describing "it" rather than the proper adverb describing the "going" of it.

But that's just being super grammatically correct.

It depends on who is asking, if it is a friend that can be trusted tell them how it is really going, good or bad. If it is other just a non discript semi positive response will suffice, like 'fine; or 'just fine, how about yourself'. Most of the time it is just a meaniless greeting and the asker does not really care or want to know anything specific. In other words it is just a variation on hello.

Typically people expect you to say, I'm fine and you?

They generally don't really care about details of your day and are generally trying to be friendly or polite.

Turn it back to them - like this maybe:

"Things are fine with me. How about you?"
