Ya know, a lot of times it's the kind of woman you approach. For example, the kind of woman who is conceited and not really that caring, will make you look like an idiot no matter what your approach. A caring woman would treat you decently whether she is interested or not.
The worst you could do is come up with some corny line. Other big turn offs are bragging, huge ego, interested in only one thing, condescending...stuff like that.
Some good traits are easy going, caring, good sense of humor, confident (not conceited), and smart.
A great way is to find a way to be introduced, then just join in on the conversation and talk about things that you both find interesting. Other great openers are just the simple ones like,"Hi","Would you like help with that?", "Can you believe that?"
Always act yourself because if she gets to know you the actor, then she will not like the real you and pretending gets old fast and both of you would be miserable.

In my opinion, you have to be yourself. Just go up to a woman, give her a nice smile and say hi! If she finds fault in that, then you are approaching the wrong woman. Find someone who has the same interests as you. Join a club; do some volunteer work -- whatever will put in the vicinity of women who have the same interests you do. Also, don't discount friendly introductions. My husband was a friend of a friend. We hung out in the same circle. We felt comfortable around each other. That helps, too.

don't try... just BE. A huge turn on to me is having a guy come up and just talk to me, no lines, no Mr. Smooth, just Mr. Real.
Just walk up with confidence, like it doesn't matter if she talks to you or not, and say 'Hi.'
Besides, if you walk up to a woman, and she gets all uppity with you, then move on to the next. Why waste your time on a woman who can't even be friendly? Just because you talk to me, doesn't mean we're suddenly committed to each other... Women like that give women like me a bad name in the dating world and it really irks me!
Confidence is the key... BUT don't be cocky...
