You can get mild sedatives. I used to get them when I lived 300 kilometers form home and drove back every other weekend so the cat wouldn't freak out. She meowed ALL the way back but otherwise stayed sedate.

Put plenty of newspaper in the bottom of the carrier so it's easier to clean if he does have an accident, and a piece of fabric (a t-shirt or something similar) that smells of his favourite person. Take spares of each so he doesn't have to travel back in his own poo. (Obvious, sorry).

Make sure the carrier is securely wedged into the car, ideally not too close to the engine noise.

Cover the carrier with a blanket. Cats are more confident in the dark. If you can, get somebody he likes to go with you, sit near him in the car and talk to him on the way.

There's a few things you could try. One thing he might not like is the cat carrier. The day before I take my cats to the vet for their yearly check-ups, I set out their carriers so they can get used to them. Before you know it, they're inside them sleeping! As far as the mess he makes in the carrier, don't feed him anything for at least an hour before you leave. If he's that nervous, anything that's in his belly will come back up. Water included. Try to make the carrier as comfortable as possible, too. Put an old towel in the carrier to make it softer for him, then lay some newspaper on top of it just in case he does have an accident. Then throw in his favorite toy. Having some familiar in there with him might help calm him. Another thing you can try is taking short rides with him before you have to take him for a long distance. This might be a way for him to get used to be in the car. Good luck!

Well you killed my idea but while you are at the vet ask about getting some sedatives. Otherwise while its gonna be a pain to keep cleaning try taking your cat on more rides kind of desensitization

Sprinkle catnip inside the carrier. Place a several towels you don't want in the bottom of the carrier and if the cat should have an accident, just throw the top towel away.

The best way to transport a cat is in a pillow case.

They can not see what is going on. Put the cat that is in the pillow case on the floorboard of the vehicle. The cat will try to move around but won't be able to get very far. You might want to put him on the back seat floorboard so that he can't move enough to get under your feet while driving.

Your cat knows what the carrier is for and once you put him in the carrier he probably goes crazy at that instant. Where if he was in a pillow case his fears may not be as bad.

I can think of two possibilities.

One is Rescue Remedy. It's a human product available in many health food stores and places like that. It works transdermally so you can put a drop (just a drop!!!!) on your finger and rub it gently inside kitty's ear flap before you leave for the vet.

Another possibility is Feliway. You can find it at pet stores. Get a dispenser and put it in your car.

Rescue Remedy costs a lot less that Feliway, and if it doesn't work on kitty you can take it yourself...

Go to WalMart. My g/f bought a car carrier that is black with a handle and a strap. It is the kind that you zip up and has a window for them to look out. Just leave it open in the house all the time like we do. Ours go in it all the time and when it is time for the Vet, they both love it. I think that it was like $20-$30. Not for sure as I was out of town when she bought it.

Try leaving the carrier open where he can go in it for a few days before the apointment. And you might want to get tranquilizers for him. Some kitties simply don't ride well.

Good luck!

Call some different vets in your area. Some vets will come to your home for animals that are afraid of travel and/or other animals.
