You need to register:
How can a biological parent register identifying information?

A biological parent whose child was born in Pennsylvania may file a Biological Parent Registration Identification Form with the Department of Health.

Forms may be requested by calling the Division of Vital Records at 1-877-PA-HEALTH or (724) 656-3100 or by writing to:

Division of Vital Records
