It is basically a refund of taxes paid on phone bills that were found to have been collected illegally.

If you are a dependent, you can't claim it. Otherwise, if you had a phone between 2003 and 2006, you would have paid this tax and can claim the refund.

The amount depends on household size. For one person, it's $30 - goes up as high as $60 for four or more people in the household. If you find all your phone bills, you can add up this excise tax if it's listed separately, and claim the exact amount if it's more. That's probably not worth the effort.

There's a line on the 1040,1040A and 1040EZ to claim it. It's down toward the end, in the "Payments" section.

If you're not filing a return but can claim the tax, there's a special form for that, 1040-T.

umm I'm not sure if your talking about this but I got something in the mail yesterday and in 2004 I didnt make a lot of money and I claimed taxes so now they are giving me back 2 hundred and change. I think if you qualified they'll mail u something
