I want to create a document where you can click on a bax and select the entry from aselectin, i.e. Mr/Mrs/Miss/Ms.

I'd also need a free format choice as well.

Can anyone help?

you can do it in Microsoft Word from the Office XP package. In word you can use activex controls to create forms with multiple choice or drop down lists. It's very easy to understand by reading the word help file on active x controls. The forms using this option may not work on many browsers so do it if you want the respondents to answer it using microsoft word.

Word 97 onwards allows you to use ActiveX commands, one of which is a drop down list box. Open the Control toolbox by using the View menu and checking the Control Toolbox box. Using help will talk you through it.

2003 version has it as well. Basically you want to create a drop down menu. I believe that this is found in tools. It is the form field check box menu.
