These kind of things should be learned at home since young... going to lessons isn't goint to help unless those behaviours are part of the routine at home.

Sitting once a week like a monkey with a teacher and eating with a fork and knife is not helping if at home she is allowed to slouch on the couch and scarfs down a sandwich while watching cartoons, cleaning her fingers on the t-shirt and drinking from the carton of milk.

They used to have etiquette schools called "charm school". You can look in the yellow pages for that. There are also videos nowadays.
