It depends on the airline. More and more they are charging for an extra seat if you are obviously over the size of a normal seat. Southwest is one of the most adiment. They will charge you at the ticket counter, but if there are more than enough empty seats by the time you board, you will get a refund. Most major airlines will over look it and not say anything, its really a touch subject.

Many people of size carry their weight differently, so its hard to sa, but you will definately need a seat belt extension. At my heaviest I was a size 26 and barely could buckle the belt, but I refused to ask for an extension. I worked for the airline and was too embarassed to represent an airline I couldnt fit on. I did however lose almost 100lb and the seats are still a tight fit, but the belts go on much easier and I can cinch them down considerably on most airlines.

Midwest has the widest seats. If you can, pay for the upgrade, First Class is always much more comfortable anyway. But, each airline and aircraft is configured differently. If you are on a regional jet like ASA, Comair, American Eagle, or any Express flights....those are small regional jets and the seats are even smaller than mainline aircrafts and you can forget it, you'll need the 2nd seat. And again, its a touch subject with many airlines, but more and more they are not afraid to start charging for the second seat. On WestJet, their seats only have a width of 17" across and that is about the industry standard, so its the same on most airlines. Unfortunately they are a single cabin aircraft, so no possiblities for an upgrade or better seat on them.

If they dont say anything to you, dont sweat it, most wont. But if they do, you need to be prepared to pay for the fare that is available that day, that is just worst case scenario. Airlines have the right to refuse anyone transport. When you check in, ask if there is a row where a middle seat is or wont be taken. They can move you, but its never guarenteed that the flight wont fill up or someone wont be assigned that seat.

Good luck, you should be ok, but that is the long and short of it. Just ask for an extension when you board and they will hand one to you. They keep them in the closet or overhead bin closest to the jump seats, so a flight attendant can access them easily if you ask before you sit down.

I don't know what size 28 equals in body size, but I have a 56" waist, and get by with a seat belt extender. The flight attendants are always very respectful and discrete about it.

I would contact the airline or a travel agent, tell them your measurements and find out for sure, as your weight may be distributed differently than mine, as I am just over 6 feet tall.
