There is no Federal Agency....you will have to file a complaint with your Department of Insurance in your state. Insurance companies are regulated by the state, not the feds.

Without knowing the specifics of your case, you'll want to make sure you go through every avenue you can within the insurance company you chose to put your insurance with prior to taking it to the state (as others have pointed out, insurance companies are regulated by the state, not the feds). Having a clear complaint in which you detail out the steps you took and the problems you had will help get a response from the state...turn in a messy complaint and you probably won't get much of a (or any) response.

Insurance is regulated on a state by state basis (for now anyway), so you will need to contact your state's insurance department with your complaint.
However, before you do, you will want to make sure you exhaust every avenue of appeal within the company first.
Make sure you appeal their decision to higher level managers until you are satisfied that you can go no further....THEN go to the State Insurance Department in your state.
An insurers responsibility to the consumer is clearly spelled out in your state's insurance laws....before you make a complaint....you might also want to check those out to make sure you are correct in your complaint.

There IS no federal oversight agency. Every state regulates it's own insurance. That's why policies vary from state to state, and rules do, too.
The HIGHEST authority, is your state insurance commissioner.
