I feel your pain Michael!!
Try thinking of things that she might enjoy discussing ahead of time because it's harder to think stuff up while you're on the spot. Practice conversations about those things in your head or privately, like rehearsing sortof, so when you're involved in the actual conversation it will feel more familiar to you. Try asking open ended questions like "What do you think about ....(a movie, teacher, whatever)" and try to avoid yes/no questions like "Do you like ..." Listen to what she says instead of thinking of the next thing your're going to say and then respond to what she says.
Remember to keep your body relaxed. People can't see what is going on inside your head, and if you keep your body relaxed you will look at ease.
And it sounds silly, but remember to breathe. When we get nervous we start to breath shallowly and it cuts off oxegen and causes us to freeze up more.
