I got a 45,000 settlement and did not pay taxes on it but it was paid out weekly for over a year. Not 4 sure if I should have included it or not. Its been 4 years and not a peep from IRS.

Workman's Comp isn't taxable if it became all medical. in case you gained a judgment for discomfort & suffering, that section might want to be taxable. Your settlement ought to state the distribution of the money.
