Doing this is likely to damage the enamel, or outer part of the tooth. If you damage the enamel you're more likely to get cavities. The substance they use in the dentist office is 1. designed to not hurt the tooth enamel, and 2. only used 1-2 times a year.
Try an anti plaque rinse prior to brushing, and a toothpaste for sensitive teeth.

If toothpaste is 'practically little sediments that brush against your teeth and help take off plaque' why would you bother adding grains of sand to your toothpaste?
It sounds to me like the toothpaste is already doing what you want the sand to do.
Just stick with toothpaste. Over-brushing your teeth will strip the enamel - imagine what sand will do.

No....It won't help remove plaque or stains but it might cut your gums up if you brushed too hard, and I'm sure it will taste awful. Do you like that taste in your mouth when you get your teeth cleaned and their being polished, the gritty taste and feel? Sand would taste a lot worse. The stuff we use is flavored too! Stick to stain removing toothpaste and proper brushing techniques and you'll get lots better results. Good luck!

I think that would probably wear the enamel off of your teeth, making them more sensitive and susceptible to cavities. Probably not a good idea.

that would rub off the enamel on your teeth, actually making them more vulnerable to cavities and bacteria.

I wouldn't recommend it as sand is quartz and your body wouldn't be able to digest it and it possible would cause damage on it's way through.
