I honestly hope someone can help you, but I don't think there are too many solutions. When my 12 year old pom started doing this, after being housebroken for years, and then continued to do it until she died at 16. I was heartbroken to lose her, but it cost me a fortune to replace the rugs and underpads. I tried to put her in a crate, but when I got home and took her outside she still came in and went on the rug. Then as she got older and became sick there was the vomiting. She had been hard to train from the beginning and in old age just went back to peeing whenever and wherever. I loved CoCo, but I will never have another dog. The vet bills were outrageous for her, but the vet could not come up with a reason for her behavior.

If she is going on the rug, she may not be feeling very well. If this is something unusual, she needs to see a vet. Animals that go on the floor normally do it because something is wrong whether it's an illness or behavior problem
One thing I could suggest to try to stop the problem is show her what she has done and scold her for it, then put her outside, there is also a treatment that you can get at Petco, Petsmart or another pet specialty store that will help get rid of the odor and discourage her from using the area again, it contains an enzyme that animals cannot stand the smell of.

I rescued a Pekingese that was not fully house trained and she was around 5 at the time we got her. She was a sneaky dog when she did it. Only when we left the house or were sleeping. So we have learned to put her in a cage at night and when we leave.
Older dogs are harder to train but it IS possible to do with vigilance. Don't listen to the idiots telling you to put the dog down. That's ignorance. Patience is virtue! Dogs want to please. You can reward the dog for all good behavior.
Good luck!
P.S.- DO NOT RUB THE DOGS FACE IN THE MESS AND HIT IT ON THE NOSE!!! A dogs sense of smell is 100 times more stronger than a humans. That dog can smell it from the other side of the room, if not the house! If anything, clean up the mess, take the towel to the yard, and lead the dog to it to show it that's where it needs to go.

by letting the dog go in the house on newspaper, youre saying its ok to go in the house, PERIOD. it is not. make the dog go outside on command by taking it out 45 mins after food or water, control when it eats & drinks. also, the vet has sprays which are odor neutralizers, should help.
if it has an accident, say NO, wipe it up with paper towel and put the dog in a confined space with it for 10 minutes. it will associate going in the house with that and not like it. put the dog in a crate at night and when you are not home. they usually will not go where they sleep. but you must also make sure it is going outside enough, older dogs have to go more often. also check the state of health, it could have an infection.

when you catch her doing it put her face near the mess, tap her on the nose and in a firm voice say NO. I found the real trick is to leave them alone and feel unwanted until they come up to you for attention. Then smuther her with love and kisses to let her know you still care. Also try Bitter Apple. It will not stain your carpet, but it will leave a smell there that she will not like.

Have you had a vet exam recently? Sometimes with older dogs they become a bit incontinent, and sometimes there are medications that can help. If that's not it- have you done anything lately that might make her mad and she's doing this out of revenge? Anything new and out of the ordinary that would be a big change for her? Hope this helps!

12 years old? Well, you might want to take it to the vet and make sure it doesn't have a bladder infection or another problem that would cause these accidents...Good Luck...

I would switch to pee pads.
go back to the begining. keep her in one room with the pee pads in there with her and keep moving them to where you want it to go.
