It can be a simple as a food allergie, an environmental allergie, or high blood pressure.
My fiance had high blood pressure back at only 16 yrs old, and it lead to kidney stones. While he was there, the attending doctor made the surgery waite until the blood pressure stablilized. Then, the doctor checked Pat's parathyroid's which were tied in to the kidney stones and had developed into benign tumors. So, they had to remove the tumors, stablize the blood pressure, then did the kidney stones surgery.
14-15 yrs later, Pat had a minor stroke associated with the headaches that were warnings of the high blood pressure. He went through therapy, got well, but did not take any meds. 1 yr and 8 months later, he had a major stroke (at only 31 yrs, 9 months old), which left him in a wheelchair today as a paraplegic (right side is severly damaged).
I would say to check out the allergies, but also the blood pressure. He may be young, but these things to happen. PS - Pat was adopted, so he had no genetic history to refer to.

I think he has to go to the doctor. My dad had sudden very painful headaches that came on for no reason, and he was diagnosed as having cluster headaches. He gets really bad ones, but the tablets from the doctor have helped. My dad didn't suffer with nosebleeds though so it;s best to get it checked out.
I don't think it can be a tumour though - when my dad went to the docs he was obviously worried that he had some sort of tumour, his gp told him that 99.9% of the time a tumour presents itself through various other ways - i.e loss of vision, paralysis, rather than just headaches. They sent my dad for a scan just in case and his came back clear.
The thing is, if he doesn't go to the docs he'll stress and then possibly make the headaches worse.

I would strongly advise seeking specialist help, there is a possibility that this may be due to blood pressure or possibly raised intercranial (i.e. inside the head) pressure. Other less potentially serious problems may be causing this, depending on where the headaches are, how long they last etc. However, a simple blood pressure check and a look into his eyes with an opthalmoscope should eliminate the more serious diagnoses. Any severe, sudden headache is cause for concern, though it could have many factors, from eyestrain (on the computer too long) to migraine to a reaction to medication.
But if it's couple with a nosebleed it is very, very serious. You must get him to the doctor or call the doctor in to see him (this only works if you live in the UK). It could be a heart problem or even a brain tumour. He could have extreme variations in blood pressure, it could shoot up which would account for the pain and bleeding.
Or he could have a blockage in the carotid artery. Any way, it is very serious and must be seen to.

Yes, it is cause for worry.
Headaches are caused by any number of things from simple stress or hunger to more serious things like blood clots in the brain and aneurysms.
If he doesn't have a history of migraines or any spell of headaches in the past, I would urge him to see a doctor immediately.

Yes he needs to see a physician. Doesn't want to go?
Possible on set of sudden headaches includes brain tumor and aneurysm (a weakening of an artery in the brain).

but he could be at risk of having a stroke if he does not get

Yes.
That's not normal.
Not wanting to go to the doc isn't necessarily a 'macho' thing.
He's probably already scared, better not to scare him any more (fellas are REALLY bad at being sick), try to get HIM to decide that HE wants to see the doc.
And whatever you do, DON'T nag!
