I too do not understand the cloth part, but after applying the cream it will NOT be gone overnight. The itching with scabies is due to an allergy to the mites themselves not their movement. The cream will kill the mites, if applied correctly, but it will take up to 2 weeks for the itching to stop, as it takes that long for your skin to break down the mites and remove them from your skin.

I don't understand the cloth part. I do know you have to get a cream from the Dr. and wash everything you come in contact with at home. Especially bedding and towels. It will be gone overnight.
