HTN - usually refers to Hypertension or Hypertensive

HLP - usually refers to Hyperlipidemia or Hyperlipidemic

CAD - usually refers to Coronary Artery Disease or Coronary Atherothrombotic Disease

Any of those acronyms can refer to several different conditions. I picked the most common, but they are all also related to conditions that effect the vasculature and heart.
