Origin: All originally Slavonic settled areas in Europe. This adds Eastern Europe, part of Germany, areas in the old Hungaro-Austrian monarchy and the Balkans to Your list. A name with Slavonic language origins & a number of spellings. The morpheme vich, wicz, witz = AFAIK son of ( witz is an older German transcription ). The vowels and the endings can vary much. I know a German named Stuttkewitz. Statkewicz has possibly lost an i due to use in a locale with a Germanic intonation ( US, Germany? ), transcription as ch instead cz for the end phoneme is common in Austria (German).
Depending on the religion of the person You can try to narrow the guess: nationality / origin. Almost all West-Slavonic nations that lived between Poland and Germany adopted German as mother language if they were not Roman Catholics but Roman Catholic areas did so too. Orthodox Christians and Ashkenazi Jews come e. g. from Ucraina, Russia. Hope this helps.

Polish (spelt like Statkewicz indicates it's been Germanised a bit) -- wicz, vitch, vich, etc. all originally meant 'son of' in a Slavic language.
