After thinking about it and talking it over with my wife, I've decided to consult a psychiatrist to help me with the self-confidence issue, anxiety and worry that threaten to ruin my work life and hurt my relationship with my wife.
I have entered into therapy once before in college. It was free and the councilor was not a licensed therapist. I want to go to a psychiatrist now for advice and to see if I have a mental issue that might require medication.
We plan to handle this through the insurance I get from my job, so do I just start calling offices and see what works with my schedule? What accommodations might I have to make for this?
And as far as the visits, what should I expect from these doctors? What will they want to know about me? What questions should I be prepared to answer?
As you can probably tell, I'm a little nervous about this experience even though I've done it before. Any advice would help a lot.

OK, it looks like I'm having some problems with my terms. I don't really know the differences between the different kinds of doctors.
My health insurance (I think) allows me to go to a psychiatrist or a psychologist. I want a doctor who I can talk to, who will offer me advice and who will tell me whether or not I should go on medication. They don't have to be able to prescribe it themselves, but I'd like a professional's opinion as to whether this would work for me.
