Yep it does! If you drink a can of caffeine free coca cola and then try coke with caffiene, you can tell the difference. However they may not actually use it as a flavoring, caffiene is addictive. If you're addicted to caffiene you'll probably buy more coke.

Not sure as I drink tea without it, and the taste and smell are the same,

no there are caffeine pill they have on taste or smell
