Most commonly it is:
o C/C++ with DirectX (and sometime OpenGL)
o 3D Studio Max (and Character Studio) / Maya
o Photoshop
o Knowledge of 3D fundamentals (ie. Trigonometry) if a 3D engine programmer.

Most game companies end up making their own (or licensing) game engines. These engines save time for the developer so they can get the game out sooner. These engines are commonly done in C/C++. Popular game engines sold to the industry include the Unreal 2007 Engine, and Quake 4 Engine.
