I have to put my strengths and weaknesses as a learner, whats the best way to do this?

I suppose alot of applications forms ask this and the one I'm filling out is for getting into a school! Please give me some tips on how I can do this successfully without either boasting/not giving enough credit to myself! EXAMPLES ARE APPRECIATED...Thanks!

strengths are easy, weakness should be something like workaholic, over prepares, things that are not really weaknesses, don't sell yourself short
