You've asked the wrong question; you've asked "Where can I send my kid off to?" when the question ought to be "Should I send my kid off at all?"
The industry as a whole is unregulated, usually unlicensed, often in serious legal trouble, and full of abuse. The bottom line is that places that will take your kid are simply not to be trusted.
There's a whole lot of mythology about these places, saying "Your kid will come back good as new!" or "You'll get your old kid back!" This is a fantasy. What really happens is bitter recriminations, anger, hatred, serious hard drug abuse as a method of buffering horrible memories caused by psychological abuse, complete destruction of self-esteem, and complete lack of respect for you as a parent.
I strongly recommend that you rethink your decision before you get into something you never wanted to be in.
