Heart attack and/or stroke is highly likely. High blood pressure may be causing the dizziness. shoulder and arm pain is another sign of heart attack. The nose bleed is also a sign of high blood pressure. Get this person into an emergency room and/or doctor as soon as possible. Don't wait, get this person into a medical professional as these issues need to be addressed right NOW!

It seems not a stroke. Get the advice of a Doctor and ask him to leave smoking immediately.

Even if it isn't stroke, see a doctor ASAP! NONE of that is normal and should NOT be ignored! Even if it's just ends up being anxiety.
