Generally, three miles of the coast is considered international waters. Responsibility for the vessel rest with the country whose flag it flies under.

They may call upon any vessel or Navy to come to there aid and it is appropriate for them to respond

Well depending on where it happened, usually there is a coast guard from a country that is close by.
