Some children have very vivid imaginations, and to them their friend is "real" though I would guess deep down they probably do actually know they are not. If the child is trying to get out of taking responsibility for their actions by blaming spills or naughtly behaviour on an imaginary friend, then yes, they know full well that it isn't real.

My youngest daughter used to have an imaginary friend named "Katie" She went with us every where, in the car, out to eat, vacation. There were several times I explained Katie wasn't real, she insisted that she was real. So I went with it.
I read that a child with an imaginary friend usually a thinker. They are more creative and have more talent in the performance arts.
If your your child has one, I wouldn't worry about it to much. Its perfectly natural. If it becomes a problem and they are acting out and they blame the invisible friend then I would worry.
If you believe in Ghost, and Spirits some people think that is what the child is playing with. I am skeptical of this opinion.
Good Luck

Depends what do you exactly mean by imaginary friend.A child(6-7 years old) which has liked for example Jennifer Aniston and wants her role(Rachel in 'Friends') to be his friend, then the child perfectly knows the difference and understands that his friend is not real. But if you mean the case when a child is talking and playing with...somebody...then I don't know because I haven't experienced it but they probably don't believe if they are not some clinical case.

You might think I am way out there, but they say the children can see ghosts. Their "imaginary" friends are real, you just can't see them. I had a friend named Carrie, my daughter has a friend named Casey. Encourage their imaginations, it's not going to hurt anyone.

Sometimes a lot of children know their imaginary friends arent real some are very covinced that they do. A lot of kids make them up to get attention. I made one up when i was a kid to get attention it only lasted for a day i got tired of narrating its actions lol.
