As with many disasters, the Chernobyl accident was the result of a confluence of multiple factors, including bad design, poor technical training, and unusual operating conditions. Any one of these could have stopped the disaster -- if the operators were better briefed about the test they were performing that day; if they did not override or disable all the safety measures that would have automatically prevented the overheat/overpressure condition that blew the lid off the reactor vessel; if the system was designed with better safeguards, light-water moderated without the (flammable) graphite reflectors and control rods that were lowered into the core rather than raised from below; and if the operators were better trained to understand exactly what was happening in the core to cause the effects they were seeing, all would have contributed in at least limiting the effects of the accident.
