Small pets can ride in the cabin under your seat in an approved carrier. Larger animals ride in the cargo compartment, also in an approved carrier. Two years ago when I brought my cat home (from South Carolina to Oklahoma), it cost me $75, one way. You will also need to check on required certificates from the veterinarian.
In the past you could also pay for a 1st class seat for larger dogs. I don't know if that practice is still allowed. Show animals sometimes traveled this way.

Where she rides depends on how big she is. If she's 25 lbs or less, she can fly in the cabin with you as carry-on luggage. If she's any bigger, she must be checked as cargo.
Airlines have restrictions on flying pets--for example, pets are not allowed to fly over four hours non-stop without provision for food, water and potty breaks, and they are not allowed to be flown to or from any airport where the ground temperature is over 85 degrees F. They also must be flown in an airline approved carrier (cannot be leashed or free roaming, or tucked in a purse or something). Most carriers are airline approved, and our Scottie has his own soft-side carrier with a cushy bottom (I think they're called Booda Bags and are available at PetSmart,eg).
Yes, it's possible, just make sure that you've booked accomodation at a pet-friendly hotel/B&B.
You shouldn't have any problem traveling throughout the Continental US, but when the plane goes over water (ie, Hawaii, Alaska, PR, anywhere international), be prepared for quarantine (quarantine is usually longer than the average vacation, making it not so practical to fly overseas with your dog).
Also make sure that you've taken her to her vet to get a travel health certificate. These are only good for 10 days, so you need to do that just shortly before you leave.

Most airlines will fly animals, but you have to call the airline. Most airlines charge a fee for animals travelling on the plane.
Be very careful!! If you can take your dog in a small pet carrier into the passenger section, do that!! The pets in the cargohold have not always been supplied with air.

The answer to that is: you don't.
She can be taken with you as "Air Freight" and you should talk to the airline you wish to use as to charges and the certainty of her making the same flight as you as well as procedures for chicking her in and picking her up, approved crates, food/water required, etc. Different airlines have different rules and listen to harrangues from different animal rights groups.
The cost is fairly high. We bought a puppy and had her sent to us air freight. It cost slightly over $200 for freight and crate.
Of course, be absolutely sure, perhaps getting something in writing faxed to you, that the hotel you will be at accepts your exact dog breed and if they have any funny requirements, like needing to show vaccination papers, not just a rabies tag and license. Be sure to have a short leash, about 4 feet long for walking and moving about. I don't even want to begin depressing you about what law enforcement does to out-of-state dogs when they bite someone and you don't need to defend a lawsuit in Maine or the bitee's home state (California perhaps, imagine flying out there 2-3 times during the suit).
Otherwise, just be smart about how you handle her in general and pro-active about her mood. She relies on you to keep her out of the sleep room at the vet's. Don't fail her.
And have fun! The more of us travelling with our pets, the easier it will be for all of us to do so in the future.

Yes some airlines allow you to ship you pets in a carrier on the plane just talk to the airline you are flying out on. I know for sure that Delta, Northwest and Continental allow you to fly your pets
