You are fluent when you can understand someone in a very noisy environment where your own knowledge of the language, nuances and etiquette allow you to fill in your own blanks and understand the speaker perfectly.

When you can lip read because you know exactly what that person would say in that situation.

You understand and can use slang correctly considering all social contexts.

You know the difference between spoken and written language as they tend to be two completely different styles.

You can understand someone else who is not fluent in the language quite well because your own knowledge can fill in the blanks in what they're trying to say.

You can speak a language at the same level as a person native to that language could.

A "native speaker" is one who speaks the language because it is his 'native language.' He is intimately familiar with it and knows the nuances, idioms and colloquialisms.

It means that you speak some foreign language as well as someone who was born in the country where that language is common.
