You will need
1) Either the original install disk, or be able to download the program from the web
2) The product key for the software programme.
If you can't remember the product key you might be able to use a programme like Belarc Advisor to get the key.

Depends on the program. If all the files and settings for it are in one directory, you can transfer it over and it will work. But most programs install files in multiple directories outside of the Program Files directory (such as dll files in windows/system32) and also add entries to the registry. As there is no way to transfer those files and settings, the program will not run on the new PC.
You normally have to reinstall the program from the original disk.

Mostly. You might trip up if there are some obscure registry entries that the program makes on initial install. Original disks are safest unless you are trying to get data off of the hard drive from a dead PC.

if you have the original discs just install them again but its maybe a good idea to email the companys help line just to check that you dont need another product key. this is likely free. :-)

hook your old hard drive in as a slave. then just transfer everything you want over to the new drive, or hell, just install the old drive (slaved) if you have space in the tower. i have three at times.
how? go here: http://tech.yahoo.com/gd/adding-a-second-internal-hard-drive-to-your-pc/2988
or maybe as simple as use a usb connection. got one? http://en.wikipedia.org/wiki/USB
