because fetal effects are unknown. we don't know how much propofol will get across the placenta to the baby, and we furthermore don't know what the effects would be on the unborn baby. the answer to those questions is that the risk is very likely negligible, but for ethical reasons it's impossible to conduct a study to prove its safety in unborn babies. so the manufacturer must put that warning to shift liability on to the doctor using the drug. i can tell you from personal experience that you're right, propofol is used all the time for c-sections and there haven't been any attributable problems that i can tell.
