There are multiple specialties that deal with your problem.
After seeing your primary care physician the options are:
pain specialist,
physiatrist (Physical Medicine and Rehab),
neurologist,
then maybe orthopedic surgeon.
As always, diagnostics come first. You may need an MRI or CT scan of the affected area (regular X-rays do not give you usually much useful information).
A good physician will recommend appropriate ancillary services like physical therapy and others.

An orthopedic doctor can treat your back pain however he may just want to make money and recommend surgery. I had a painful backache. It was a kidney infection. I took a prescription for seven days. No more back ache no more pain.

Orthopedist Dr. first your Head Dr. or primary physician will check you if they think you need a specialist they will send you to a orthopedist Dr. they work with bones, muscles, ligaments you name it. Chiropractic Dr. are for alignments, no offense they know there stuff but you need a specialist, a physical therapist is going to need a prescription from any other Dr to work on you.

You may want to try a neurologist since you have problems around the neck area and also in your hands. These two may come from the same place, maybe a pinched nerve in the neck area. I would go there first and then perhaps an orthopedic Dr. Good luck...I hope you get some help.
